Health insurance registration for a child adopted in Québec

Responsibility : Régie de l’assurance maladie du Québec

To register a child adopted in Québec, call the Régie de l'assurance maladie du Québec for the required form.

You must fill out the form and return it to the Régie with one of the following documents:

• the original adoption order


• the original placement order


• a letter from the clerk of the court to the effect that an adoption order was rendered


• the child's birth certificate or a copy of the act of birth, under the child's new name

Note
The Régie returns all originals, but not necessarily when it issues the child's health insurance card.
