You can choose one or more given names for your child, as well as a single or compound family name. A child’s family name must be formed from either parent’s family name or both.
It is recommended not to give your child more than four given names, and to write the usual given name (the one used in daily life) in the appropriate box of the Declaration of birth form.
Note
The given name of a parent cannot be used as a surname.
