Your child's birth must be reported to the Directeur de l'état civil (DEC) within 30 days. To that end, you must fill out the Declaration of birth form given to you by the hospital or birthing house and send it to the Directeur de l’état civil.
In addition to establishing your child's identity by stating his or her family name and given name(s), the act of birth indicates citizenship and the parents' names.
Note
An act of birth is necessary to obtain certain documents, such as a health insurance card, social insurance card or passport. It also serves to prove entitlement to certain government programs or social benefits.
